In-home support for medically fragile children through Georgia’s Pediatric Program.

We provide in-home care coordinated through Georgia’s Pediatric Program (GAPP) for medically fragile children, working alongside your child’s care team and case manager.

What types of services are available?

Care is coordinated with your GAPP case manager and may include:

• In-home personal care and daily living support
• Coordination with your child’s care team and case manager
• Assistance with routines, mobility, and daily activities
• Safety supervision and family respite support
• Ongoing communication with family caregivers

Where do I receive GAPP-coordinated care services?

In most cases, you will receive these services in your own home. Program eligibility and authorized services are determined by Georgia’s Pediatric Program.
